We provide rehabilitation assistants to clients living with a brain injury or neurological illness to support their ongoing rehabilitation and enable them to maximise their function and achieve their goals whilst living in the community.
We work closely with the multi-disciplinary team to ensure consistent approach to the client’s goal led support programme.
Our Promise
We pride ourselves in providing excellent, high calibre Rehabilitation Assistants who are experienced in brain injury. We match staff and clients, and if upon assessments we feel that we may not have a suitable member of staff, we will recruit specifically for the package.
All out fantastic staff are DBS checked and our service includes mentoring, regular supervision ongoing training of our staff.
Trained & Experienced Staff
All our Rehabilitation Assistants are experienced in working with clients with brain injury or neurological illness and have a background in Psychology or Mental Health.
All staff have had an enhanced DBS check and undertake ongoing training and professional development specific to brain injury.
The progress of staff placement will be regularly monitored during staff supervision to ensure rehabilitation goals are being met and support is meeting your needs.
Clinical Documentation
Our Rehabilitation Assistants provide individualised sessional reports for medio-legal purposes, which are monitored by the Director and depending on client need, are shared regularly with case managers.
Goal-Led Intervention
Our Rehabilitation Assistants work collaboratively with the client, family and multi-disciplinary team to ensure that the intervention and support provided is closely linked to the client’s own goals.
Our Process
An enquiry via email or telephone call is made
We will gain as more information as possible about the needs.
A referral form is sent
A referral form will be emailed to your chosen address by one of our office team.
A follow up within 24 hours
You will receive a phone call from our team to check you have received the referral form and answer any question you might have at this stage
Referral form and supplementary information is received
Once the referral form is received by our office team; the information is reviewed, and this is then used to commence the support plan and begin to identify potential suitable rehab assistants
An initial assessment meeting is arranged
An initial assessment is arranged with the referrer and the director, Claire. This assessment allows for further information to be gatherer to ensure that the rehabilitation plans are built putting clients at the center of their support plan.
Suitable staff are then matched based upon information provided
Taking all the information gathered from the referral form, supplementary information, and initial assessments. A suitable rehabilitation assistant will be matched with your client.
Initial meeting with the client and staff member
This is an informal meeting which give the client and rehabilitation assistant a chance to meet, to ensure that they are suitably matched and are happy to work together.
Research has shown that positive working relationship leave to better outcomes.
Package confirmation, acceptance form completed and commencement of service.
