THE INFORMATION WE HOLD ABOUT YOU
Personal information, or personal data, means any information about an individual from which that person can be identified.
Personal contact details such as name, title, date of Birth, addresses, telephone numbers and personal email addresses.
Photographs
A log of our communications with you by email, telephone and WhatsApp
Diagnosis, Date of Injury,
Family/Friend Name, address Phone number, Relationship to Client
Litigation Solicitor name, Deputy’s name
Type of Instruction – Joint instruction/claimant instruction, funding availability
Invoice contact details
Litigation friend
Any medical information you provide for example, legal and medical reports.
HOW WE COLLECT THE INFORMATION
This information will have been provided, or will be provided, by you or we may receive your data indirectly from the following sources, yourself, deputy, solicitor firms, GP, Hospital, family and other case management companies. The relevant information is used to provide our services to you in our capacity as a Case Management and Capacity Assessment company on a need-to-know basis.
DATA SECURITY
We store your personal information securely on a secure password protected computer system. Your information can only be accessed by authorised members who have access to your information (on a need-to-know basis). They understand their legal responsibility to maintain confidentiality and follow practice procedures to ensure this.
All information is held securely. This includes reasonable physical, administrative and technical safeguards to protect to prevent your personal information from being accidentally lost, used or accessed in an unauthorised way, altered or disclosed.
We have put in place procedures to deal with any suspected data security breach and will notify you and any applicable regulator of a suspected breach where we are legally required to do so.
YOUR RIGHTS
Your duty to inform us of changes It is important that the personal information we hold about you is accurate and current. Please keep us informed if your personal information changes.
